ExactCare and Tabula Rasa HealthCare combined in November 2023 to become one of the nation’s leading pharmacy care management companies that specializes in caring for people with the most complex, chronic needs. This new brand name is an important milestone in our integration and unifies our companies both internally and externally. HomeFree Pharmacy Services officially became part of the AnewHealth brand in July 2024, combining with ExactCare, an AnewHealth pharmacy. 

Patients will still know us as ExactCare Pharmacy, although they may see some visual updates to our brand and logo. How they take their medications remains the same. We will continue to seamlessly provide high-quality pharmacy services to our patients.

ExactCare provides comprehensive pharmacy services designed to meet the unique needs of people who take multiple prescription medications and have chronic conditions. Often, the people we help need assistance with activities of daily living (ADLs); have experienced recent SNF stays, hospitalizations or ER visits; see multiple prescribers; or have recently used home health services.

ExactCare provides members with an easier way to stay on track with their medications. We provide pharmacy services that enable optimized medication therapy; improved medication adherence; lower healthcare utilization; and better overall health.

Your member will complete a personalized health and medication review with an ExactCare representative. This is a virtual visit. This time will be used to capture medication and prescriber information, important health details that will help our pharmacists, and a Social Determinants of Health (SDoH) screening.

We typically complete the health and medication review within two business days of receiving the referral, if not sooner. The time it takes for members to receive their first medication delivery will depend on a variety of factors, including the date they last refilled their medications.

ExactCare does not charge for our services, including packaging, monthly delivery and clinical support. Our members are responsible for only their prescription copayments, determined by their insurance plan, as well as the cost of any over-the-counter medications they order that are not covered by insurance.

ExactCare accepts most major insurance plans, including Medicare Part D, Medicaid and private-pay insurance.
