Living with a chronic health condition can be stressful. There is a lot for patients to understand about how to manage their symptoms and take care of themselves. Even taking new medications can be overwhelming when they first begin.
The good news is, the patient’s healthcare team can help them. By working together with their doctor and pharmacist, patients can gain a greater sense of control over their situation and chronic condition.
Here are some tips for patients on how to play a more active role in their care and help them get the support they need.
1. Listen and Learn
An individual’s doctor and pharmacist are a great source for learning about a specific condition and medications. They are qualified to offer health advice and can often make recommendations tailored specifically to the patient.
2. Communicate Often
Whether someone has a question about a medication they are taking or have experienced a new symptom, it’s important they let their team know! The more information they share, the better they can fully understand their needs. Patients should be sure to speak up when they have concerns about their care or think they may be experiencing a medication side effect.
3. Follow the Treatment Plan
This will be the most effective way for a patient to maintain or improve their health. If the patient struggles to follow their plan, they can talk to their doctor or pharmacist, who can work with them to make adjustments, if appropriate and/or needed.
4. Ask a Family Member or Caregiver for Support
Sometimes it helps patients to have someone close to them involved in their care. This is particularly important if patients have trouble remembering information or reading instructions. Patients should not be afraid to ask for help if they need it!
